About the Role
We're looking for a Care Partner to coordinate resident care and service requirements, working closely with residents and/or their representative to ensure their needs and wellbeing is met. As Care Partner, you'll ensure that all areas of support for residents and surrounding networks are considered and contacted when organising pre-entry clinical assessments, move-in support, care planning, delivery, and evaluations, referrals, accessing and documenting externally funded care. Care Partners play a critical component within the overall Care Team, liaising with a wide range of internal and external stakeholders while sharing their knowledge and past experiences to build up and support fellow staff. Role Responsibilities
- Coordinate the delivery of safe, high-quality, evidence-based care through assessments, planning and evaluations that are person-centered and aligned with regulatory and organisational standards.
- Build and maintain strong relationships with residents and families.
- Interpret care options and funding arrangements for residents.
- Connect residents to health and other services as needed. Collaborate with general practitioners, nurse practitioners, pathology services, allied health professionals, hospitals, pharmacies and government agencies.
- Liaise with the rostering team to ensure services are delivered as planned and in a timely manner.
- Ensure timely and accurate execution of all documentation in accordance with LDK Policy and Procedure and as directed by the Care Partner Manager.
- Complete ‘Move In’ assessments and care planning.
- Provide daily, weekly and monthly updates to the Clinical and Assistant Care Manager.

Qualifications/Certificates/Professional Experience
- AHPRA registration
- Bachelor of Nursing
- 2 years of experience working in a clinical setting
- Experience in the community care sector/with home care packages
- Experience working as a member of a multidisciplinary team
- Aged care nursing experience (Desirable)
- Experience in a team lead or management position (Desirable)
