About the Role
We're seeking a confident individual who can oversee the coordination of care services for our village residents living with dementia at LDK Seniors' Living. As Dementia Lead, you'll be supporting and motivating your team to make a positive difference, initiating quality improvement in dementia care while upholding the LDK values of Love, Decency and Kindness. You'll strive to ensure flexibility and individualised care towards our residents, with a focus on independence and enablement.

Role Responsibilities:
- Identifying and addressing the clinical, health and social care needs of residents living with dementia, ensuring independence and choice.
- Lead with a positive and calm demeanour, while training and mentoring others accordingly.
- Be responsible for incident and complaint reporting and management.
- Manage ongoing reviews and assessments of residents, liaising with allied health, General Practitioners, and Nurse Practitioners.
- Manage family discussions, and multidisciplinary meetings where required.
- Lead staff through clinically relevant engagement and psychosocial support strategies for the person living with dementia.
- Coordinating aged care assessments through ACAT/MyAgedCare and navigating Commonwealth funded programs.

Qualifications/Certificates/Professional Experience
- Registered nurse with current AHPRA registration.
- Demonstrated experience working in Dementia Care.
- Experience in the community care sector/with home care packages
- Aged care nursing experience (Desirable)
- Experience in a team lead or management position (Desirable)
